Patients with acute MI who fail to be diagnosed on the basis of clinical presentation, ECGs, and lab tests (false negatives) are also ill-served. Wheels on the sand: controversy and compromiseIn this subset, the first step toward establishing the diagnosis is a high index of suspicion. The diagnosis for many of these can be established with an electrocardiogram or a series of ECGs obtained as the infarct evolves. And for decades this was the only modality available other than the history and physical.The latest entrant into the field is an immunochemical assay based upon some clever monoclonal antibody technology. The procedure employs two monoclonal antibodies, one specific for CK-M and the second specific for CK-B.Photographers drool over the picture possibilities. Like chameleons, sand dunes change color as the day progresses: pale gold at dawn, glaring white at midday, rosy at sunset. Shooting at daybreak or sundown best captures the richest tones and the longest shadows. Carry camera gear in a tightly sealable bag to keep sand out.Early travelers saw dunes as desolate obstacles. In the late 1800s, surveyor George Wheeler described a traverse of Death Valley: "The route lay for miles over light, white, shifting sand. The stifling heat, radiation, and constant glare from the sand were almost overpowering."The harsh dune environment winnows out the weak. Plants must be tenacious enough to survive high winds (40 mph or more), abrasion, burial, drastic temperature ranges, incessant sun, and meager rain. Long taproots (40 feet deep on some species) and extensive root systems help plants cling and absorb scant moisture. Dunes actually retain subsurface water, and while the top layer may register an infernal 140 degrees on a summer day, just inches below it may be a cool, moist 60 degrees.Vehicular access remains a controversial issue at the Panamint Dunes near Lone Pine, California: they were closed to vehicles in 1980, but 500 acres are being considered for reopening by permit after requests from recreation groups and Inyo County officials.Methodology is critical to CK isoenzyme testing.3 Electrophoresis is the most popular and also is the reference method. But several "quick and dirty' assays have been advocated to assist in rapid diagnosis of acute MI, for electrophoresis is tedious and not available in most labs on a Stat basis.The first antibody is bound to a solid phase--sich as a plastic bead--which in turn is incubated with the serum to be tested. The solid phase binds both CK-MM and MB, isoenzymes sharing CK-M activity. The solid phase is then incubated with the second antibody, which is specific for CK-B. A "sandwich' is formed only with solid-phase bound molecules of CK-MB. The second antibody is conveniently coupled to an alkaline phosphatase marker (although other markers could be substituted), making detection of MB activity simple through use of traditional laboratory procedures.On the other hand, the coronary atrtery occlusion may have occurred several days previously, and CK-MB activity since peaked and disappeared. In this case, only the flipped LDH1 and and LDH2 fractions will establish a laboratory diagnosis.Children know instinctively what to do: they run, jump, slide. In open, unvegetated areas, you can try sliding on a lightly inflated truck inner-tube, snow dish, or thin plastic mat. If you have a castoff pair of downhill skis, apply silicone spray to the bases and try dune skiing. Don't do this with good gear: It's about as abrasive as skiing on sandpaper. Tenacious flora, adaptive faunaFailure to establish the diagnosis also precludes medical therapy and life-style modification programs designed to ameliorate positive risk factors for future vascular catastrophes: diabetes, high blood pressure, obesity, smoking, and insufficient exercise.Off-road vehicles are barred from seven of the dune systems we list. In the other areas, such vehicles have limited access or are allowed to range the dunes. Self-propelled explorers may find this vehicular presence intrusive. While most dune drivers follow the rules and regulations, those who stray into vegetated areas can cause more than visual disturbance. Some ephemeral plants--short-lived annuals that grow, blossom, and set seed in a brief time span--die out if disturbed. Desert plants take decades to regenerate; when they're crushed or the soil mantle is broken by tires, the delicate balance of the dunes is upset. Even the natural contours of dunes can give way: as vehicles cut trails through vegetation, blowouts occur, and the sand fans out through the cut.Over sometimes hundreds of years, grains of sand are blown overland, bouncing along not far off the ground, before piling up against a barrier, perhaps a mounting. At coastal systems, such as the Oregon and Pismo dunes, ocean current and tides deposit sand on wide, flat beaches where it is banked and held by prevailing winds and vegetation against natural backstops. The four distinct dune shapes shown above testify to the varied work of the wind. Leeward sides (slipfaces) of dunes are never steeper than 32 [deg.] to 34 [deg.], called the angle of repose--the maximum angle at which loose sand remains stable.In lieu of electrophoresis, these alternatives employ chromatography, immunoinhibition, radioimmunoassay, and immunochemistry to quantitate MB activity. Although these assays have fairly rapid turnaround times, none provides the correlation to the reference method demanded by clinicians. So the search goes on for the ultimate CK-MB procedure.It should be noted that not all CK-MB is biologically active. This immunochemical method will detect both active and inactive forms of the molecule. Thus the assay detects the mass of CK-MB present rather than its biologic activity. (The fact that an alkaline phosphatase marker is employed should not confuse one into believing that he is measuring biologic activity of an enzyme.)As the map at top right indicates, these big dunes--from nearly 4 to 40 square miles--are scattered over the West. We list 13 of the biggest and most accessible dune systms on the opposite page. Even if you visit just one of them (or stop by a smaller coastaul (dune) on a day or weekened outing, you might get hooked. Some aficionados can never get enough time hiking, sliding, and photographing these compelling formations.Now through April is perhaps the most clement season to visit most these dunes. The inland ones are in arid, almost rain-free regions offering clear, crisp winter days. You can hike for miles without gasping for water or bracing yourself against gritty blasts. (Late spring often brings high winds; summer brings savage heat.) And several are on or near routes to popular getaways: Palm springs and Las Vegas. The two coastal dunes we list take winter rain straight in the face, but between storms they offr sparkling vistas. How nature stockpiles sandSo now the laboratory diagnosis of acute MI is a reality. Or is it? Among patients with possible or probable acute MI whose ECGs are equivocal, how well can the lab rule in or rule out the diagnosis? This questions among patients increasing frequency by clinicians and pathologists alike, for it is important to diagnose the smallest acute MI at its earliest stage, while avoiding the significant implications of a false-positive diagnosis. The analogy to the diagnosis of early pregnancy or early malignancy should be apparent.

Since an elevated CK-MB is not, per se, an indication of cardiac muscle damage, an elevation is meaningful only activity is concomitantly of MB activity is concomitantly increased. It remains to be seen whether a biologic assay or a mass assay that measures both active and inactive forms to total CK will provide thebest denominator in detecting when the concentration of CK-MB is elevated.

Magnificent sand seas of the West


In languid grainy waves, seas of sand flow over parts of our landscape, cresting and tumbling into deep troughs. The West has some of the largest and most remarkable dune systems in North America, from coastal strands stretching 7 miles long to inland hills peaking at 700 feet.


Since prodding up a dune can be tiring, consider going in the morning, when sand is still hard-packed and a bit moister. Sand is usually firmer on the windward (stoss) slope than it is on the steeper slipface. If you can't start early, wait until late in the afternoon, since winds commonly pick up at midday as the temperature rises. It's wise to wear sunblock, a wide-brimmed hat, and sunglasses to protect against reflected sunshine, and a bandanna and long-sleeve shirt to shield you nfrom blowing sand. High-topped boots will help keep sand out. Always carry a supply of water. In desert dunes, watch for rattlesnakes and scorpions.If no MB activity is demonstrated, it may be because the infarct is of such recent onset that insufficient time has elapsed to detect its presence in the blood. Serial CK-MB determinations are required in this instance to establish a lab diagnosis of acute MI.How high must concentrations of MB activity be before they are considered pathologic? Concentrations greater than 20 IU per liter are definitely elevated. There is a gray zone in the range of about 12-20 units, and values less than 12 units are observed among normal individuals without either cardiac or voluntary muscle damage.The diagnosis of acute myocardial infarction poses no problem in a patient who presents with shock, diaphoresis, an ashen gray appearance, and crushing substernal chest pain radiating down the left arm. Anyone who has once witnessed this classical presentation will be able to make the diagnosis in the future.CK-MB activity is present in the serum of normal individuals, albeit in small concentrations. The critical question is where does one place the cutoff between normal and abnormal concentrations, knowing that overlap exists between normals and patients who have recently sustained acute MIs?About 20 years ago, enzyme panels were introduced to confirm or exclude acute MI among patients with atypical presentations whose ECGs were equivocal. Enzymatic confirmations consisting of CK, SGOT, and LDH were employed initially. Characteristic time-dependent elevations of these enzymes would confirm the diagnosis, and approximate date, of a presumed acute MI. But false-positive CK elevations resulting from intramuscular injections, and nonspecific elevations of SGOT and LDH, limited the usefulness of these panels. About seven years ago, this triad was replaced with an equally sensitive but much more specific panel consisting of total CK and LDH as well as isoenzymes of each.1, 2Today, the image of dunes as barren, lifeless places has changed, and the growing appreciation of these sand-bound ecosystems is reflected in fervent efforts to protect them. Offroad vehicles have been banned from some preserves and restrictued from entering parts of others. In the troughs between the dunes (interdune areas), you'll find scurf-pea and "blowout" or bunchgrasses, and at dune edges, taller shrubby plants like creosote bush, four-winged saltbush, honey mesquite.Certain conditions are required for dune building: an abundant souce of dry, fine- to medium-grain sand (.125 to .5 millimeters); wind that blows in a consistent direction and with sufficient strength (at least 10 mph) to move sand; and an opposing wind or land barrier to halt the sand.Regardless of the percentage of CK activity represented by the MB fraction, the absolute concentration is also significant but does not necessarily reflect damage to mayocardial tissue--all striated muscle contains small amounts of MB activity. Therefore damage to any striated muscle will elevate CK-MB, although such elevations are not pathognomic of acute MI unless the percentage of MB activity is simultaneously increased.Why is the West so dune-rich? Aridity is one major factor in the formation of our inalnd dunes. Another factor in many areas is the ample supply of building material: sand deposited in river-beds or eroded from cliffs. On our coast, ocean waves pound sandstone and granitic cliffs and deposits the lightest grains on wide beaches.Many animals have made special adaptations to their arid, sandy world. The kangaroo rat drinks no water but absorbs moisture solely through the seeds it eats. The sand treader camel cricket and fringe-toed lizard have feet that act like snowshoes.Nocturnal creatures such as snakes, beetles, and mice abound in each dune system. These animals survive by burrowing below the surface or hiding under brush by day, then emerging at night to hunt or forage. Tiny crisscrossing tracks tell of journeys and chases in the night. In the morning, look around shrubs for tracks before the wind erases them.Then there are the false positives. These relate principally to definition of terms and to methodology, although gross and random error obviously occur as well.Two criteria are employed in this regard: the absolute concentration of CK-MB activity in enzyme units per liter of serum, and the percentage of CK activity expressed relative to the total CK activity. CK-MB activity that is less than 3 per cent of the total is not considered pathologic, while a concentration greater than 5 per cent certainly is. A 4 per cent cutoff designed for high-sensitivity, low-specificity must be balanced against a 5 per cent cutoff, which provides higher specificity but a corresponding loss of sensitivity.But many patients with acute MI present with less than classical signs and symptoms: indigestion, persistent pain in the back or in the base of the neck, or an unexplained arrhythmia. How is the diagnosis of acute myocardial infarction then established?On coastal dunes, tough, salt-tolerant European dune grass stabilizes the foredunes 'those nearest the waves), and back dunes are scattered with bright yellow beach primrose, pink morning glory, Indian paintbrush, and California poppy.

State of Colorado Approves Energy Fuels' Request to Defer Financial Assurance Payments


Energy Fuels Inc. (TSX:EFR) ("Energy Fuels" or the "Company") announced today that the Radiation Program of the Colorado Department of Public Health and Environment (CDPHE) has approved a request by the Company to defer its remaining financial assurance payments for the Pinon Ridge Uranium and Vanadium Mill until next construction season. This approval is an amendment to the company's Radioactive Materials License (the "License"). The original payment schedule was based on mill construction starting in September 2011. However, due to ongoing litigation on the License, Energy Fuels will not be able to start construction until at least March 2012.

This week, the Canadian Association of Medical Radiation Technologists (CAMRT) and its provincial associations launched the second phase of their award-winning rebranding initiative, the Image of Care. The Image of Care campaign is intended to change the way medical radiation technologists (MRTs) are recognized throughout the medical profession as well as by the general population, and bring a face to this often misunderstood profession.

The CAMRT is also the national professional association that represents and promotes the MRT profession and is an active participant in the Canadian health system. This award, presented in the category of public education/information, recognizes organizations that have created and distributed valuable information to policymakers, regulators, or the news media that enhanced public understanding about the efficiency, quality and safety of products and services. The initiative likely strengthened consumer confidence in the marketplace and/or may have improved government and/or public support for your organization and its members. Acart Communications, an Ottawa based Advertising agency that specializes in social issues marketing provided strategic advice and developed the creative concepts for the campaign.


The Image of Care campaign was formally launched in June at the CAMRT Annual General Conference in Saskatoon. Since then, the initiative has continued gaining momentum with the adoption by CAMRT members, with posters beginning to appear in waiting hospitals and healthcare facilities from coast to coast.

Springs woman wins 'Caring is Sexy' design contest


A Colorado Springs woman unzipped her creative talent to win an international design competition.


In the meantime, one thing she won't have to worry about is buying shampoo. As winner of the design contest, she is slated to receive $500 worth of Sexy Hair products.Sooner rather than later would be good: "I'm out of conditioner," she said.The winner was determined by a panel of judges along with an online "people's choice" vote; though Porter did not win the online vote, she was named the overall winner.Porter's design was inspired by the black, zip-up sweatshirt her mother wears each year over a pink shirt while taking part in the Avon Walk for Breast Cancer in Breckenridge; Porter's grandmother battled breast cancer.And she does get something else out of it: "She does my hair for free," Porter said of the salon owner.Cherie Porter, 32, a mother of three, was named winner of the Caring is Sexy: Design Contest, staged by California-based Sexy Hair Concepts. Her zipper design concept will appear on limited edition packaging of Sexy Hair hairspray aimed at increasing awareness of breast cancer; the special edition can will debut in 2012.Porter is self-taught, but she does want to get more formal training, she said.Porter actually had two designs chosen for the final round of competition. When she heard she had reached the finals, "I was jumping around and screaming. ... I thought that was as far as it would go."Five percent of net proceeds - up to $40,000 - from sales of the special edition can will go to Look Good ... Feel Better, a national public service program that teaches female cancer patients beauty tips to look good and feel better about their appearance during chemotherapy and radiation treatments.
